Before you get started

A few helpful answers.

What services do you offer?

Pique Medical provides personalized primary and preventive care, hormone and midlife evaluation, medical weight management, and selected wellness services. Specific recommendations depend on your evaluation.

What should I book for my first appointment?

Choose a new patient visit on Book Now. You do not need a separate initial appointment type for every concern. Explain your priorities in the intake.

Do I need to join a membership?

No. Visit-based care is available. You can discuss the optional Core Care membership after reviewing your needs.

What does Core Care cost and include?

The current Core Care membership is $149/month and includes up to one visit per month, ongoing primary care follow-up, preventive planning, chronic condition support, secure messaging for brief questions, and priority scheduling. See the DPC page for details.

Is membership insurance?

No. It is a direct arrangement with the practice for specified services and does not replace health insurance for hospitals, emergencies, specialists, and other outside care.

Are medications and labs included?

Not automatically. Medications, pharmacy or shipping charges, labs, imaging, supplements, and procedures are separate unless explicitly included in your agreed care plan.

Do you bill insurance for office visits?

Office visits are self-pay. Insurance may be used for eligible outside labs, imaging, or prescriptions, subject to your plan and the participating provider.

Where do I find current visit fees?

The Book Now page lists current appointment options and standard visit fees. Program-specific charges can differ and should be confirmed before payment.

Where do I go for lab testing?

Labs are ordered through outside facilities. Follow the instructions on your order for the appropriate facility and any preparation. Pique Medical does not provide routine in-clinic blood draws.

Can my visit be virtual?

Yes, when clinically appropriate and permitted by state licensure rules. You must be physically located in an eligible state during the visit. Confirm eligibility when booking.

Are prescriptions guaranteed?

No. Treatment decisions follow clinical evaluation. Additional information, testing, an examination, or specialty care may be needed.

How do I request an approved medication refill?

Use your established patient instructions and the approved refill options on Book Now. If your symptoms or treatment needs have changed, schedule a medical visit.

How do I contact the practice between visits?

Use the existing patient portal or secure messaging instructions provided to you for clinical questions. Text (815) 373-0283 for scheduling help. Do not use ordinary email for sensitive clinical information.

How do I book tox or filler with Raquel?

Use the separate nurse-injector booking link on Book Now. That service has its own booking system.

Where are you located?

24 W. Division St., Manteno, Illinois 60950. Check your appointment confirmation for details.

Is messaging an emergency service?

No. Scheduling, portals, and messages are not emergency services. Call 911 or go to the nearest emergency department for an emergency.
